UIC Completion Reporting
What is a completion report?
A completion report is a record of the physical and administrative details of a well. The completion report will include crucial information such as:
Physical Record
- History of workovers performed on a well
- Well location (surface and bottom)
- Size and depth of strings of casing
- Cementing reports
- Formations encountered
Administrative Record
- Well type: Injection or Producing
- Well status: Active or Shut-in
- Lease ID & Unit #
- Well number
Why are completion reports required?
The completion report is the official document used to:
- Record the as-built completion of the well;
- Confirm that a well’s construction matches what was applied for in the injection permit application;
- Verify that all permit conditions are met;
- Determine which rules apply to that well (producing vs. injection, etc.).
When is a completion report required?
Completion reports must be filed before a well begins injection and after every workover. Even if no changes are made to the well’s operation, a completion report must be filed to record that work was done. For example, if the packer is pulled and reset, that must be reported on a completion report even if the packer is set back at its original depth.
Where are completion reports filed?
Completion reports are filed on Form W-2 and Form G-1. Form W-2 is used for wells in oil fields and Form G-1 is used for wells in gas fields. Both forms are submitted online through the RRC Online portal.
How is a completion report filled out?
The completion report is written with a production well in mind, so some of the terminology and wording doesn’t exactly match what would be expected for an injection well. The following sections are intended to help fill out a completion report for an injection well.
Spud Date & Date of First Production After Rig Released
The spud date is the date the work began. For injection wells, the date of first production is asking for first date of injection after the work was completed.
Potential Test Data
The potential test data on page 1 of the online forms does not apply to injection wells. Skip page 1 for all injection well completion reports.
Date Plug Back, Deepening, Workover or Drilling Operations Commenced & Ended
These are the start and stop dates for the work that you are reporting on the form. If no physical work was done and the form is being filed for an administrative reason, this date will become the effective date of the administrative change (for example: the date the well is became an injection well or the date the well became active.)
Well Type
The well type indicates how a well will be used. There are four well types on the completion report. The injection well options are in bold.
- Producing
- Shut-In Producer
- Active UIC
- Shut-In UIC
Purpose of Filing
The purpose of filing indicates what part of the well record is being changed. There are 7 purpose of filing options on the completion report. The injection well options are in bold.
- Initial Potential
- Retest
- Reclass Oil to Gas
- Reclass Gas to Oil
- Reclass Injection to Producing
- Reclass Producing to Injection
- Well Record Only
Type of Completion
The type of completion indicates what type of work was done. There are 6 options, all of which could be applicable to an injection well.
- New Well
- Deepening
- Plug Back
- Sidetrack
- Re-Entry
- Other/Recompletion
